Players that are going to join a team or possibly change divisions in the PGRHL may be asked to complete an HHQ and every new player that is joining the Rec. D Division and the Co-Ed Division (Novice) must complete the attached hockey history questionnaire, and return via email to the league administrator. email@example.com

Choose a destination such as your desktop or a folder you can easily access and save the pdf. Now open the pdf “Hockey History Questionnaire” from the saved location. From the navigation bar at the top click on tools and select “Typewriter”. Then click into the boxes and complete the questions. Once completed save the file titled as follows with“Your Name” Hockey History Questionnaire, and email it to admin.
Anyone who intentionally provides false or inaccurate information could be disqualified from playing in the league.

We have updated the Rules for player recruitment for the Masters A division, and Rec. D division. The guidelines for Masters B are currently under review and will be updated shortly to reflect changes made to MB divisional allignemnt. These rules were adopted by each division over the past couple of years or were approved at the executive meeting on September 4, 2013. They have since been updated as of July 18, 2015. We have clarified the language and the intent of the rules.

If a player play as an unregistered player without handing in a completed form or plays more than 1 regular season game without registering or renewing the team will be fined $100 per incident. Registration is a condition of our liability policy.
